Replace Your Social Security Card
For an adult US citizen:

Get your documents together to prove your identity.
Birth certificate is not acceptable as a primary form of ID.
Get your:
1. U.S. driver's license
2. State-issued non-driver identification card
3. U.S. passport and show it within 10 days.

1. Certificate of Naturalization, Certificate of U.S. Citizenship.
2. Employee identification card/badge
3. Medicaid card (issued within the last two years)
4. Health insurance card (not a Medicare card)
5. U.S. military identification, U.S. Government identification card
6. School identity card, certified record, or transcript (current year)
7. U.S. Indian Tribal card (Social Security has to approve as an acceptable ID)
8. Certified copy of a medical record
9. Life insurance policy.
Only originals or certified copies by the issuing agency are acceptable.
No photocopies or notarized copies of documents.
Some documents can be used to prove both citizenship and your identity.
To prove citizenship,
You would use your U.S. birth certificate or U.S. passport or Certificate of Citizenship or Certificate of Naturalization.
You will need a Social Security Card Application form.

The first 4 pages are instructions.
If you are 12 years of age or older, and have never received a social security card and social security number, you must apply in person to one of the social security offices.
